Caring Caller Programme
Enhancing the health and wellbeing of Welsh communities is at the heart of everything we do at St John Ambulance Cymru, and as part of that we’re proud to run a programme called the Caring Caller. The programme has been specifically developed to help assist and be the first port-of-call for patients who have been discharged from hospitals and used our Patient Transport Service (PTS).
Our Caring Caller contacts patients who we have transported home to ensure they are safe, well, and coping outside of the hospital environment.
They will ask the patient if they require any additional information or support and signpost them to services which could help them and support their recovery.
Sometimes, our Caring Caller might visit a patient in their home and provide support and guidance if requested.
In addition, our PTS crews can refer a patient direct to our caring caller if they are concerned or worried about a patient and their general welfare after they’ve taken them home on being discharged from hospital.
Our Caring Caller will liaise with our safeguarding team and discuss any concerns raised. From here, any necessary action will be taken.
